Kidney transplant is the most performed transplant surgery throughout the world. With the advent of newer immunosuppressive drugs and induction agents, the rejection rates have decreased leading to longer survival of the kidney grafts and recipients. Xenotransplant and graft tolerance are the much awaited, but possible looking landmarks in coming years. Multiorgan donation from high KDPI (extended criteria donors) and DCD (donation after cardiac death), HCV donors help bridge the gap between demand and supply of the organs. Kidney transplant is in its most exciting and established era.
The Indian perspective of kidney transplant faces many challenges. We do not perform more than 15,000 transplants/year in comparison to 1 lac new CKD patients added to the waiting list yearly. Still, most of our kidney transplants are by live donation. Our deceased organ donation rate is still below 1 per million population. Although kidney transplant remains the domain of urologists, many urology centers lack a kidney transplant program. Thus, the newly trained urologist grossly misses basic training in kidney transplant and vascular access. Ultimately that will reflect inadequate centers offering transplant services in 2 tier and 3 tire cites. Despite the current rise in dialysis centers, adequate number of transplant centers are still lacking in India.
The basic concept of writing this book is to provide basic and practically oriented tips regarding kidney transplant. We feel this book helps students and freshly passed urologists and nephrologists to have a better understanding of kidney transplant. This book is an attempt to cater to this need. It covers many practical aspects of transplant including immunology, donor nephrectomy surgeries (open, laparoscopic, robotic approach), recipient surgery (open and robotic), how to start this program at your institute, transplant surgery armamentarium, medicolegal aspects, pediatric transplant, anesthesia aspects and many more. Simple but methodical way of chapter layouts and very expressive approach of the most experienced transplant surgeons in this field shall help budding urologists to aspire to be kidney transplant surgeons. Editors and publishers have done their best to bring out the most recent and relevant facts of kidney transplant, but we are very much open to suggestions and improvements.

Dr. Ravindra Sabnis MS, MCh (Urology) is current Chairman and Head, Department of Urology, Muljibhai Patel Urological Hospital, Nadiad, Gujarat. His urology journey started from Lokmanya Tilak Municipal Medical College (LTMMC), Mumbai in 1989. He has served urological society by serving many posts: Past Secretary and President, west zone USI, Past Secretary and President, USI, Past Chairman, Board of Education, Deputy Director- Research-Urology Association of Asia, Member, SIU Academy, and Member, Education World Endourology Society. He has keen interest in kidney transplant and has been associated with renal transplantation for last 30 years. He was appointed as convener of transplant subspecialty section of national board of examination and now is accredited as teacher for renal transplant fellowship program, FNB of NBEMS.s
Dr.Sishir Garg MD, DM, DINB (Nephrology) is a distinguished leader in the field of nephrology, serving as the Chairman, Department of Nephrology, Muljibhai Patel Urological Hospital, Nadiad, Gujarat, a premier center of excellence in India. With an illustrious career spanning over more than three decades, Dr Gang has made significant contributions to both clinical practice, teaching and medical research, solidifying his reputation as an expert in kidney health and disease management. Dr Gang's nephrology academic journey began with DM specialization from Christian Medical College (CMC), Vellore. He has been nominated to pivotal positions including trustee of the Indian Society of Nephrology (west zone chapter), past President, Indian Society of Nephrology (west zone chapter). He has authored numerous peer-reviewed articles and book chapters, addressing a wide range of nephrological issues, from chronic kidney disease and dialysis to transplantation and innovative therapeutic approaches.
Dr. Jigish Vyas MS, DNB (Gen Surgery), DINB (Urology) has done urology training from Muljibhai Patel Urological Hospital, Nadiad in 2010. Thereafter he worked as an Assistant Professor, Institute of Kidney Disease and Research Center (IKDRC), Ahmedabad and again at Muljibhai Patel Urological Hospital (MPUH), Nadiad till 2014. He pursued private practice in Anand, Gujarat till 2023. Currently, he is doing an abdominal transplant fellowship at Atrium Health Wake Forest Baptist, North Carolina, USA. He has keen interest in renal transplant, multiorgan recovery and fistula construction for vascular access. He presented many papers in various national and international conferences related to transplant. Before pursuing his further training in abdominal transplant in the US, he was instrumental in establishing transplant programs in and outside the state. He was an invited transplant faculty in ISOT meetings and also conducted many transplant related workshops in MPUH, Nadiad. He worked to raise awareness regarding organ donation in society.
Dr.Abhijit Konnur MO DNB (Nephrology) is a Senior Consultant, Nephrologist, Muljibhai Patel Urological Hospital, Nadiad, Gujarat. He heads the Community Dialysis Program at MPUH. Dr Konnur's academic journey began in 1993 with his medical degree from Calicut Medical College, Kozhikode and Diplomate in National Board, Delhi, followed by advanced training in adult nephrology and renal transplantation in Canada. A prolific author, Dr Konnur has penned chapters in four authoritative textbooks on nephrology, which are widely used by medical students, residents, and practitioners. He has authored numerous articles in esteemed national and international peer-reviewed journals, contributing to the advancement of disease, dialysis, and renal transplantation. He has a particular interest in the management knowledge in areas such as renal proteomics, genetic renal disorders, chronic kidney of complex renal conditions, including glomerulonephritis, polycystic kidney disease, and diabetic nephropathy. Dr Konnur is also a dedicated educator and mentor, actively involved in training the next generation of nephrologists. His passion for teaching and mentorship has inspired many young physicians, fostering a culture of excellence and continuous learning within the department.
Dr. Abhijit Patil Ms. MCh (Urology), DNB (Urology) is a Consultant, Urologist and Transplant Surgeon at Muljibhai Patel Urological Hospital, Nadiad, Gujarat. He did his urology training from BYL Nair Hospital, Mumbai and was university topper. Following which, he did Fellowship in Endourology-Laparoscopy-Robotic from Endourology Society Inc, USA. He is currently pursuing FNB renal transplant fellowship at Nadiad. He is also co-editor of renal transplant subspecialty newsletter. He did his Urological Society of India awarded-MIUC international travelling fellowship in department of transplant and vascular access of NHS Sheffield Hospital, UK. He has contributed previously to many national and international book chapters. He has indexed national and international publications to his credit and is a Publon academy approved peer-reviewer. He is currently council member of Indian School of Urology and USI-Collaborative Research Cell. He was council member of Youth Organization of USI (YOU 2022-2024), He is also council member of USI-west zone.
