About the Book Cardiovascular disease is the main cause of death across Europe, accounting for over four million deaths each year. The main objectives of preventive cardiology are to increase the duration and improve the quality of a patient's life, and this indispensible manual provides practical advice on how to structure and run a preventive cardiology programme, addressing the prevention of cardiovascular disease in clinical practice through lifestyle, risk factor and therapeutic management of patients.
The approach described in this manual is for patients with atherosclerotic cardiovascular disease, those at high risk of developing cardiovascular disease and also for their families to provide the appropriate support. It is written by the multidisciplinary team who coordinated the EUROACTION demonstration project in preventive cardiology in 8 countries in Europe, and provides a comprehensive lifestyle and cardiovascular risk factor management strategy for all high risk patients and their families. It is directed at doctors, nurses, dietitians and physiotherapists, physical activity specialists and other disciplines involved in the preventive and rehabilitative care of these patients.
The manual takes the professional through the patient and family pathway - from patient identification, recruitment of patient and family, comprehensive assessment of lifestyle and cardiovascular risk factors, to management of lifestyle change: stopping smoking, making healthy food choices, becoming physically active, achieving a healthy weight and shape and management of blood pressure, blood lipids and blood glucose to healthy levels. A programme model is described which includes both group and individual one-to-one approaches to care. The role of each member of the multi-disciplinary team is described together with the tools available to achieve lifestyle and risk factor change.
Catriona Jennings is a cardiac specialist nurse and the European Research Nurse coordinator for EUROACTION and EUROASPIRE III in the Department of Cardiovascular Medicine at the National Heart and Lung Institute, Imperial College London, UK based at Charing Cross Hospital. She read French for her first degree and subsequently trained as a nurse at Charing Cross. She then specialized in cardiac nursing and finally cardiac rehabilitation, leading a programme at Stoke Mandeville Hospital in Buckinghamshire. She was responsible for coordinating the EUROACTION project in eight countries and 24 centres in Europe from its launch in 2002. She led the multidisciplinary London team who were responsible for training project staff and developing the health professional and educational materials. She coordinated the third EUROASPIRE survey in 22 countries in Europe. She is currently writing her PhD on concordance for cardiovascular risk factors in families.
Alison Mead was the EUROACTION dietetic coordinator and is a research assistant in the Department of Cardiovascular Medicine at the National Heart and Lung Institute, Imperial College London, UK based at Charing Cross Hospital. She is also a chief Dietitian at Imperial Healthcare NHS Trust. She has worked in clinical dietetics for 12 years, specialising in cardiology for 7 years. She lectures at Imperial College and Chester University. She is a committee member for the UK Heart Health and Thoracic Dietitians Specialist group of the British Dietetic Association Group. She has recently published the updated UK guidelines for diet and secondary prevention of cardiovascular disease. Alison represented the BDA for the post MI NICE guidelines and is a co-expect member of the Cardiovascular Risk NICE guidelines. She was awarded the Role of Honour from the BDA for her contribution to dietetics and is now doing her PhD in Nutrition and Cardiovascular disease.
Jennifer Jones is the EUROACTION physiotherapy coordinator. She also coordinates the Masters in Preventive Cardiology at Imperial College, London. For five years preceding this she was a clinical specialist in cardiac rehabilitation at Harefield Hospital in Middlesex. She is past co-chair to the national group the Association of Chartered Physiotherapists in Cardiac Rehabilitation (ACPICR) and is still actively involved in its activities to develop exercise and physical activity provision for cardiac patients and their families. She is also represents physiotherapists in preventive cardiology and rehabilitation nationally in the British Association of Cardiac Rehabilitation Exercise Professionals Group (BACR-EPG) and is an assessor for a national cardiac rehabilitation exercise instructor qualification. Jenni completed her MSc in 'Applied Human Science - Exercise and Health'. She is now doing her PhD in physical activity in preventive cardiology.
Annie Holden is the EUROACTION physical activity coordinator. She has lectured, assessed and verified courses in Sport, Exercise and Fitness throughout her career. She also works as the Partnership & Active Lifestyles Brand Manager for a local Leisure Trust with strategic responsibility for borough-wide health and fitness provision which includes physical activity for special populations and referral programmes. She has delivered community based exercise programmes for over 15 years. She represents exercise specialists nationally within the British Association for Cardiac Rehabilitation - Exercise professionals Group and is a member of the BACR Phase IV Steering Group. Annie's completed a Masters in Nutrition, Physical Activity and Health. She is the manager of the EUROACTION UK programme - 'MyAction' in Bromley - a national initiative to promote a nurse prescriber led community based multidisciplinary preventive cardiology programme for hospital and general practice.
