About The Book

A woman in Iowa has her eyes screened for diabetic blindness by a camera no doctor reviews. A teenager confides in a chatbot that has no duty to call for help. A surgeon conducts an operating room in which most of the instruments are guided by models she did not build and cannot audit. In each room there is a second one: an artificial system doing the work the credentialed physician used to do.

Yoram Friedman, a physician who trained in those rooms and then built the platforms now displacing it, argues that medicine's professional wall was doing two jobs, not one. It controlled who got in. It also, quietly, governed the care that happened inside. AI has dissolved the wall, and almost no one has noticed that the second job went with it. The patient is still in the room. The thing that used to watch over her is gone.
