Die hypertrophische obstruktive Kardiomyopathie (h. o. K. ), zuerst 1907 anhand zweier Sektionsprotokolle von SCHMINCKE als "diffuse Hyperplasie des link en Ventrikels, besonders im Bereich des Ausflugtraktes" beschrieben, 1910 durch BERNHEIM mit einigen weiteren Fallen erganzt, geriet bis 1952 in Vergessenheit, als DAVIES liber eine Familie mit mehreren Todesfallen berichtete. Aber erst 1957 gab Sir RUSSEL BROCK die erste detaillierte Be- schreibung des Krankheitsbildes. Seither ist ein ausgedehntes Schrifttum liber diesen Krankheitsbegriff unter stark wechselnden Bezeichnungen ent- standen und eine Zahl von einigen hundert einschlagigen Fallen mitgeteilt worden. In der vorliegenden Monographie wird diese Zahl urn 47 Patienten vermehrt, welche nach dem neuesten Stand kardiologischer Untersuchungs- methoden identifiziert und liber langere Zeit klinisch und ambulant be- obachtet und behandelt wurden. Diese kritische Studie wird durch instruk- tive Abbildungen und Tabellen nach jeder Richtung hin dokumentiert. Die h. o. K. fand sich in jahrelang konstantem Durchschnitt von ca. 2% aller Herzkatheterisierungen. Das h: rankheitsbild ist augerordentlich varia- bel. Es gibt sichere angeborene wie erst im spateren Leben auftretende Falle. Die Genese ist ungeklart. Langfristige Beobachtungen lassen eine Progredienz und Verkiirzung der Lebenserwartung erkennen, also Grund genug, sich mit dies em Leiden eingehend zu beschaftigen. Der Begriff der "idiopathischen hypertrophischen Subaortenstenose" trifft nur einen, aller- dings wichtigen Teilaspekt. Die genaue Diagnostik erfordert kardiologische Spezialmethoden. Das klinische Bild mit Dyspnoe, Herzklopfen, Schwindel, Synkopen und Angina pectoris ist vieldeutig. Von der einfachen Herz- hypertrophie lagt sich die h. o. K.
