In Deutschland wie auch in anderen westlichen Industrienationen stellen Herz Kreislauferkrankungen mit Abstand die haufigste Todesursache dar. Ihr Anteil betragt etwa 50070 aller Todesursachen, davon entrallt etwa ein Drittel auf Korona re Herzkrankheiten und Herzinfarkt [58]. Nach Angaben des Statistischen Bundesamtes kam es im Jahr 1987 allein in der Bundesrepublik zu ca. 137000 Todesfallen durch ischamische Herzkrankheiten, dies entspricht einer Mortalitatsrate von 224 pro 100000 Einwohner. In der glei chen GroBenordnung liegt die Zahl der zusatzlichen nichttOdlichen Herzinfarkte, die in vielen Fallen zur fruhzeitigen Invalidisierung fUhren. Die Pravalenz obstruktiver Koronarerkrankungen in der klinisch unauffalligen, d. h. symptomfreien Bevolkerung, wird auf 4-7% geschatzt [16], so daB in Deutschland ca. 3 Millionen Einwohner yom Eintritt eines symptomatischen oder auch stummen Herzinfarktes bedroht sind. Die durch Mortalitat und Morbiditat an ischamischen Herzkrankheiten verursach ten Gesamtkosten wurden fUr das Jahr 1980 auf 7,5 Milliarden DM geschatzt [14]. In den afrikanischen und asiatischen Entwicklungslandern, aber auch in Japan ereignen sich ischamische Herzerkrankungen wesentlich seltener [46, 61]. Schon 1941 beobachtete Snapper, daB auch in China Herzinfarkte seltene Ereignisse sind [55]. 1973-1975 wurde die chinesische Mortalitatsrate an "Arteriosklerotischer Herzkrankheit" mit 18,9 pro 100000 Einwohnern entsprechend 2,5070 aller Todes ursachen angegeben [34]. Seit 1984 ist China in einer internationalen prospektiven Studie der Weltge sundheitsorganisation beteiligt (Sino-MONICA-Beijing Study), in der Trends der Mortalitat und Morbiditat von kardiovaskularen Erkrankungen ermittelt werden."
