In weiten Gebieten der Erde stehen sich heute zwei groBe Probleme diametral gegeniiber: die standig steigende Zahl der kinderlosen Ehen einerseits und die LJberbevolkerung andererseits. Daraus erwachsen gerade fiir den Arzt vollig neue, zum Teil noch kaum losbare Aufgaben. Die hinreichend bekannten, tra- gischen Einzelschicksale bei Nichterfiillung des Kinderwunsches konnen jedoch nicht ohne weiteres kollektiven Menschheitsproblemen geopfert werden. 1m Handbuch der Haut- und Geschlechtskrankheiten von JADASSOHN war die mannliche Infertilitat nur kurz im Rahmen der seinerzeit so vorherrschenden postgonorrhoischen Komplikationen abgehandelt. Ohne Zweifel wurden damals andere Ursachen zu wenig beachtet. Die groBe Bedeutung der mannlichen Infertilitat bei sterilen Ehen wurde erst wahrend der letzten 3 Jahrzehnte erkannt. Die Kenntnisse iiber dieses Gebiet haben sich so stark ausgeweitet und so stiirmisch entwickelt, daB inner- halb der Dermatologie mit ihrer langen Tradition auf diesem Gebiet sozusagen ein neues Spezialfach - ahnlich wie die Endokrinologie innerhalb der inneren Medizin - entstanden ist. Die von gynakologischer Seite vielfach erhobene Forderung der Betreuung von beiden Ehepartnern durch einen Arzt diirfte heute nur selten moglich sein. Eine Abtrennung des Forschungsgebietes der mannlichen Infertilitat von der weiblichen Infertilitat und Sterilitat, also eine Abtrennung der Andrologie von der Gynakologie war nach entsprechender Ent- wicklung in den anglo-amerikanischen und anderen europaischen Landern unver- meidbar. Trotz aller Mahnungen von Andrologen und Gynakologen wird auch heute noch oft die Untersuchung des Ehemannes unterlassen, der Frau aber ohne Not- wendigkeit eine Kette von differenten diagnostischen Eingriffen und mehrfachen Operationen zugemutet.
