Die seit den Anfiingen gestellten Fragen urn die Biologie und Biomechanik wer- den mit verfeinerten Vntersuchungsmethoden immer relevanter beantwortet. Wir wissen, 1. daB durch die Aufbohrung tiber den Weg der Drucksteigerung im Markraum eine Intravasation seines Inhaltes im systemischen Kreislauf herbeigeftihrt wird; 2. daB das Markfett und auch Markpartikel in der Lungenstrombahn abgefangen werden, die zwar den Lungenkreislauf und die Herzleistung vorubergehend be- eintrachtigen, aber unter regularen Vmstanden keinen Dauerschaden verursachen; 3. daB die im Schockzustand der Verletzten durchgeftihrte Marknagelung eine le- bensbedrohende Gefahrdung des Verletzten darstellt. W ozasek versucht, unter standardisierten Bedingungen im Tierexperiment Antwort auf offene Fragen zu geben, indem er - den Schockzustand imitierend - die Me- chanismen der Fetteinschwemmung wahrend der Marknagelung untersucht und die Auswirkung auf die Lungenfunktion zu definieren trachtet. Die methodisch einwandfrei aufgebaute Versuchsanordnung einerseits sowie die vergleichende Berucksichtigung der Ergebnisse anderer Autoren andererseits erlaubt einen bemerkenswerten Einblick in die pathophysiologischen Ablaufe rund urn das Schockgeschehen, die Fettintravasation, das Fettemboliesyndrom und die Mark- raumosteosynthese. Wozaseks Arbeit betrachte ich als einen bemerkenswerten Markstein. Sie bringt einiges Licht in das Dunkel, ohne es voll auszuleuchten, bestatigt Vorangegangenes und wirft neue Fragen auf: alles in allem hochst aktuell und von beachtenswerter klinischer Relevanz.
