Auf den ersten Blick mag es unverstandlich erscheinen, weshalb dem Thema "Enterale Emiihrungstherapie" eine so groBe Bedeutung zugemessen wird, daB dieser Themenbereich als Gegenstand eines wissenschaftlichen Workshops ausgewiihlt wurde, wo doch die enterale Substratzufuhr die natiirlichste Applikation zu sein scheint. Die Begriindung liegt in einer interessanten und stiirmischen Entwicklung der klinischen Emabrungstherapie. Es besteht kein Zweifel, daB die medizinische Leistungsbreite in vielen Bereichen wesentlich von der Entwicklung der parenteralen Emiihrungstherapie mitgetragen wird. Auf der Grundlage der in der Entwicklung der parenteralen Emiihrungstberapie gewonnenen Erkenntnisse sind heute Voraussetzungen erfiillt, neue Emiihrungstherapien auch auf enteralem Wege zu er- forschen und zu entwickeln. Wir miissen zunachst festhalten, daB nicht nur die Vermeidung schwerwiegender Komplika- tionen der parenteralen Emahrungstherapie maBgebend war, sondem die Erfiillung von Vorbedingungen die Entwicklung enteraler Emabrungstherapiekonzepte wesentlich stimulierte. Eine Voraussetzung zur Entwicklung enteraler Emiihrungstherapien bestand in der Unter- suchbarkeit der Funktion des Magen-Darm-Trakts unter physiologischen und pathologischen Bedingungen. Uber neue biochemische und biophysikalische Verfahren wurden nicht nur Antworten zu Fragen der Motilitat und der Energieversorgung des Darms, sondem gan;z besonders auch die hormonelle Charakterisierung und Quantifizierbarkeit der Digestions- und Absorptionsleistungen - vor allem des an sich gesunden Intestinums unter pathologischen Korperbedingungen - moglich. Die Wiederentdeckung des Magen-Darm-Trakts als Organ hatte zur Folge, daB iiber neue Technologien die Bereitstellung von Nabrgemischen ermog- licht wurde, die an die veranderten Digestionsleistungen adaptiert wurden und so weitgehend die fehlgeschlagenen Elementardiaten durch die modemen Oligopeptid- und Saccharid- praparate ersetzten.
