Sofortige Beseitigung der Streptokokken und anschlieBende Dauerbehandlung mit 1,2 Mill. E Benzathin-Benzylpenicillin (Tardocillin(R)) eimnal monatlich oder mit 0,5 g Sulfo- namiden (Durenat(R)) taglich. 1m akuten Stadium Prednisolon (40 bis 60 mg taglich) fur 4 bis 6 Wochen und zusatzlich bis zu 4,0 g Acetylsalicylsaure. Progressiv-chronische Polyarthritis: (bei Erwachsenen nicht selten, hliufigstes Manifestationsalter 40 bis 60 Jahre, bei Frauen hliufiger als bei Mannern). Voraussetzung fUr eine erfolgreiche medikamentose Therapie ist eine wirkungsvolle Kran- kengymnastik und Bewegungstherapie. Keine Chemotherapie, keine sog. Herdsanierung. Medikamentose Dauerbehandlung mit: Analgetica: z. B. Acetylsalicylsaure, Phenylbutazon, Indometacin. Prednisolon (nur wenn Analgetica nicht mehr ausreichen, und nur zusatzlich, niemals an- stelle von Analgetica) a) bei akuter Verschlimmerung, kurzfristig ca. 1 Woche hochdosiert, ausschleichende Dosierung nicht erforderlich; b) selten Dauerbehandlung erforderlich, dann Tagesdosen um 10 mg, meistens weniger, einmal taglich morgens (Achtung, Patienten verschweigen eventuell eigenmlichtige DosiserhOhung). Basistherapeutica D-Penicillamin, Gold: sorgfaltge Oberwachung, damit gefahrliche Nebenwirkungen ver- mieden werden. 30% Versager, 40% partielle Remission, 30% Vollremission. Chloroquin: wird nur noch selten verwendet; Wirksamkeit ist weniger sicher als bei Penicillamin und Gold. Cytostatica und Immunsuppressiva: nur nach Konsultation eines Spezialisten. Spondylarthritis ankylopoetica (M. Bechterew): (kann dominant erblich sein, Manner sind hliufiger befallen als Frauen). Wichtige Voraussetzungen fUr den Erfolg der medikamentosen Therapie sind taglich aktive und passive Bewegungsiibungen. Dauerbehandlung nur mit Analgetica: Indometacin (Amuno(R)): gilt bei dieser Indikation als besonders wirksam.
