About The Book

Die antipsychotische Wirksamkeit der Neuroleptika sowohl bei schizophrenen Psychos en als auch bei florider psychotischer Symptomatik im Rahmen anderer psychiatrischer Erkrankungen, z. B. bei organischen Psychosen oder bei Drogen- psychosen, ist seit langem bekannt. Demgegenuber ist der Einsatz von Neurolep- tika bei nichtpsychotischen Erkrankungen weniger gut abgesichert. Entsprechend sind derartige SWrungen fUr Neuroleptika z. B. in der Roten Liste bzw. in den Fachinformationen der Hersteller nur bei einigen Praparaten im Sinne einer "Nebenindikation" aufgefUhrt. Urn so erstaunlicher ist der Befund, daB nach Daten des Medizinischen Instituts fUr Statistik in Frankfurt, in Ubereinstimmung mit skandinavischen Arzneimittelstatistiken, z. B. fUr das 2. Quartal 1997 in Deutschland bei nur 14 % der Neuroleptikaverordnungen schizophrene Psycho- sen, hingegen bei mehr als der Halfte der Verordnungen nichtschizophrene Erkrankungen, insbesondere Neurosen (23,7 %), angegeben werden. Diese Zahlen zeigen die Notwendigkeit, auch bei den nichtpsychotischen Erkrankungen Indi- kationsgebiete, Nebenwirkungsspektrum und Richtlinien zur Praparate- und Dosiswahl einschlieBlich rechtlicher Aspekte sorgfaltig zu uberdenken. Die Breite der Anwendung von Neuroleptika bei dies en Erkrankungen in Arztpraxis und Klinik steht in einem gewissen Kontrast zu der relativ geringen Zahl kontrollier- ter wissenschaftlicher Studien auBerhalb schizophrener Psychosen. Insbesondere einige neuere Praparate, wie Sertindol, Olanzapin oder Risperidon, sind bisher fUr andere Indikationen noch gar nicht zugelassen, obwohl gerade bei den neuen Substanzen zumindest fUr schizophrene Psychosen ein besonders gunsti- ges Nutzen-Nebenwirkungs-Verhaltnis anzunehmen ist.
