Zu Ehren des 65. Geburtstages von Maurice E. MUller und J org Rehn fand vom 3.-5. 3. 1983 auf der Reisensburg die 16. Arbeitstagung zur klinischen Trauma- tologie statt. Hierbei befaJl, ten sich 46 Spezialisten aus den Gebieten der Chirurgie, Neurologie, Ortho- padie, Physikalischen Therapie und Unfallchirurgie rnit den Fragen der Biomechanik, Pathophysiologie, Diagnostik, operativen und konservativen Therapie sowie der Nach- behandlung von Spatschaden nach Verletzungen des Schultergtirtels. Die Thematik betraf sowohl die Folgen knocherner Verletzungen an Clavicula, Scapula und proximalem Humerus als auch die Spatschaden nach Traumatisierung der zwischengeschalteten Gelenke sowie der Nerven dieser Region. Die entsprechenden Probleme wurden in Dbersichtsreferaten dargestellt, die interessie- renden Fragen anschlieBend in ausgiebig und offen geftihrten Diskussionen erarbeitet. Durch die Kugelgestalt des Schultergelenkes und die Kompensationsmogllchkeiten in den Nebengelenken des Schultergtirtels stellen die rein knochernen Verletzungsfolgen funktionell das kleinere Problem der Spatschaden dieses Korperabschnittes dar. Bei ge- gebener Indikation erlauben die modernen Verfahren einer winkelgenauen Korrektur, libungsstabilen Osteosynthese und ggf. Knochentransplantation meist die Wiederherstel- lung anatomischer Skelettverhaltnisse. Weitaus groBere Funktionsbehinderungen und therapeutische Schwierlgkeiten gehen von den Weichteilschaden aus. Dies betrifft sowohl die schmerzhafte wie die schmerzarme Schultersteife, die chronischen Verrenkungen der Schllisselbeingelenke als auch die Folgen einer habituellen Schulterluxation. Die operative Arthrolyse des Schultergelenkes ist ein nlitzlicher, aber difiziler Behandlungsschritt, der eine genaue Vordiagnostik erfordert. Das- selbe gilt fur die geschlossene Schultermobilisation. Bei den chronischen Verrenkungen der Schllisselbeingelenke basiert die Operations- indikation auf einem genauen Abwagen der tatsachlichen Behinderung.
