Unter den Erkrankungen und Verletzungen im Bereich des Schultergelenks spielen Rupturen der Rotatorenmanschette eine herausragende Rolle. Sie werden hliufig verkannt und zum Ten noch immer unter der nichtssagenden Sammeldiagnose, Periarthritis humeroscapularis" in vielfaltigster Weise zu behandeln versucht. Oft dauert es Monate, manchmal sogar Jahre, bis eine Ruptur erkannt wird. Dabei kann man aus den Schilderungen der Patienten bereits richtunggebende Hinweise erhalten und eine Ruptur mit der einfachen Ma&ahme einer Schulterarthrographie nachweisen. Neben den diagnostischen Problemen sind aber auch noch manche andere Fragen offen. Aus diesem Grunde haben wir das Thema "Schmerzhafte Schulter und Rupturen der Rotatorenmanschette" als eines der Hauptthemen der 35. Jahrestagung Nordwestdeutscher Orthopaden im Juni 1985 in Kiel ausgewiihlt und vOn kompetenten Referenten darstellen lassen. Dabei interessierte nicht nur die A tiologie und Pathogenese von Manschettenrup- turen und der gegenwartige Stand der Diagnostik einschliefllich modemer Verfahren wie der Schulterarthroskopie, sondem auch die Indikationen und Methoden der konservativen und operativen Behandlungsmoglichkeiten. Besondere Aufmerksamkeit wurde daruber hinaus der postoperativen Behandlung und Begutachtung von Manschettenrupturen ge- widmet. Die zeitlich gro ziigig pemessenen, lebhaften Diskussionen wurden von uns zusammen- gef t und den jeweillgen Referaten angeftigt. So kam eine sehr breite tlbersicht zustande, die den aktuellen Stand zum Thema "Rup- turen der Rotatorenmanschette" wiedergibt. Wir danken allen Autoren fur ihre Beitrage und konstruktive Mitarbeit. Den Herausgebem der Hefte zur Unfallheilkunde, sowie den Damen und Herren des Springer-Verlages mochten wir fUr die vorziigliche Zusammenarbeit danken, durch die eine rasche Publikation der Beitrage moglich geworden ist.
