Der Autor greift dabei den pathophysiologischen bzw. in ternistischen Standpunkt zur chronischen Pankreatitis ebenso wie die technischen Moglichkeiten des Chirurgen auf und gibt eine Synopse der diagnostischen Einschatzung sowie des therapeutischen Vorge hens. Es besteht sicher Einigkeit daruber, daB zwischen Internisten und Chirurgen die Behandlung einer chronis chen Pankreatitis abgespro chen werden sollte. Hierbei geht es nicht allein urn die Frage der Schmerzbehandlung, sondern auch urn das Problem der funktions gerechten und funktionserhaltenden chirurgischen Therapie, da die verbleibenden endokrinen und exokrinen Reserven der Bauchspei cheldruse berucksichtigt und in das therapeutische Kalkul mit einbe zogen werden mUssen. So ist der operative Eingriff nicht als ein Ab schluB nach Scheitern der konservativen Behandlung anzusehen, sondern nur als ein Therapieschritt von allerdings entscheidender Be deutung im Veri auf dieser Erkrankung.
