Die Erfolge der modemen Osteosynthese sind nicht zuletzt sorgfaItiger metallur gischer Auswahl der verwendeten Metalle zuzuschreiben. Je "passiver" sich ein Implantat im Korper verhiilt, desto eher wird es ohne "physiko-chemische" Ent zlindung vom Korper akzeptiert. Diese Erkenntnisse haben dazu gefiihrt, grund siitzlich jede Mischung von Metallen zu vermeiden - ja, gar bei Anwendung kombinierter Implantate (Schrauben und Platten) gleiche Provenienz des Aus gangsmaterials zu verlangen. Empirische Beobachtungen in der Technologie und im Experiment legten die Vermutung nahe, die Kombination von austenitischen Stiihlen und Titan konnte eine Ausnahme bilden. Die ausgesprochene Passivitiit des Titans macht seine Anwendung als Implantat wiinschenswert. Seine gegeniiber austenitischen Stiihlen etwas geringere ReiBfestigkeit legt die Verwendung von Schrauben aus resisten terem Material nahe. Orientierende Tierversuche zeigten eine erstaunlich gute Ver triiglichkeit der Mischung von rostfreiem Stahl und Titaneum. Das groBe Ver dienst von Herm Dr. RUEDI ist es, diese Frage einer griindlichen experimentellen Priifungunterzogen zu haben. Es bedarf wirklich einereingehenden experimentellen Dokumentation, urn das Dogma des Verbotes von Metallmischungen zu durch brechen. Es muB aber betont werden, daB durch die schonen Versuche von RUEDI lediglich Schliisse auf Mischungen des gepriiften austenitischen Stahls mit tech nisch reinem Titan gezogen werden diirfen. Alle anderen Mischungen verschiedener Implantate sind abzulehnen. Die nachgewiesene Vertriiglichkeit der Stahl-Titan-Mischung lieB dann einen aus gedehnten klinischen Versuch zu, der im abschlieBenden Teil der Monographie wiedergegeben ist und die tierexperimentellen Ergebnisse grundlegend bestiitigt.
