About The Book

Die Bedeutung dieser Studie liegt zum gegenwartigen Zeitpunkt darin, daB erstmals biochemische Daten aus der friihesten Phase nach einem Trauma, d. h. direkt am Unfallort erhoben und ausgewertet werden konnten. Ein weiterer Vorteil ist, daB diese Daten aus der (Pra-)KIinik ebenfalls erstmalig die Interpretation von biochemischen Reaktionen zulassen, die sich bislang uberwiegend auf tierexperimentelle und/oder Laboruntersu- chungen stutzten bzw. in klinischen Studien zu einem wesentlich spateren Zeitpunkt begonnen wurden, wenn davon auszugehen ist, daB ein GroBteil der Reaktionen bereits abgelaufen oder zumindest initiiert worden ist. Die somit erreichbaren Interpretationsmoglichkeiten erscheinen in vollig neuem Licht, da die Weichenstellung flir das weitere Schicksal des betroffe- nen Patienten eben in der Fruhphase nach Trauma erfolgt und alle Rettungs- und ReanimationsmaBnahmen zu diesem Zeitpunkt durchgefiihrt werden. Folglich sind unsere Daten von groBem Wert flir die praklinische und fruhe klinische Versorgung. Mit der vorliegenden Studie konnte auch erstmals gezeigt werden, daB Para- meter, denen bislang eine hohe Bedeutung im Sinne einer Weichenstellung in der Friihphase nach Trauma zugeordnet wurde, de facto in der KIinik und damit im Gegensatz zur experimentellen Laborsituation eine geringere Bedeu- tung haben werden. In diesem Zusammenhang sind flir die ersten 24 h nach Trauma explizit zu nennen: TNFa, IL-la, IL2-R und sICAM, sVCAM. Auch die notwendige scharfe Trennung zwischen mechanischem Trauma und dessen primaren (inflammatorischen) Folgen gegenuber anderen inflammatorischen Zustanden wie generalisierte Entzundungen, Sepsis, Scho k sowie Verbrennung wird mit unseren Ergebnissen deutlich unter- strichen.
