Vom 18. - 20. Februar 1982 fand auf Schlo Reisensburg die 15. Arbeitstagung zur k1inischen Traumatologie statt. 45 Fachleute beschliftigten sich mit der Problematik von Verletzungen des Schultergiirtels, wobei in Vortriigen und ausgiebigen Diskussionen die Biomechanik, Pathophysiologie, Diagnostik und Therapie sowie die Nachbehandlung der Traumafolgen in diesem wichtigen Skelettabschnitt abgehandelt wurden. Die The- matik umf te die Frakturen von Oavicu1a, Scapula und Humeruskopf sowie die Luxa- tionen an den Gelenken des Schultergiirtels. Ein spezielles Gewicht wurde auf die Weich- teilverletzungen, insbesondere die Ruptur der Rotatoren am Schultergelenk sowie auf die Verletzungen von Gef!i&n und Nerven gelegt. Es hat sich dabei gezeigt, auch Experten liber banale und alltagliche Probleme, wie z. B. die Oaviculafraktur, ausgiebig diskutieren konnen. Die Indikationen zum opera- tiven Vorgehen bei Briichen des Schllisselbeines, des Schulterblattes und des Oberarmkopfes konnten klar erarbeitet werden. Die Osteosynthese an der Scapula setzt eine genaue Ana- tomiekenntnis voraus, auch partielle Nervenschadigungen sollten vermieden werden kon- nen. In der Behandlung der Humeruskopffraktur scheint sich eine Tendenz zur vermehrten Anwendung, halboffener" Verfahren abzuzeichnen, indem die geschlossene Spickdraht- osteosynthese aus guten Griinden wieder vermehrt Anwendung fmdet. Viel zu wenig be- kannt ist auch heute noch die Diagnostik der Rotatorenverletzungen, bier bringt die friihe Arthrograpbie eine signifIkante Entscheidungshilfe fUr aktives Handeln. Diese hliufIg liber- sehene Verletzung fiihrt in vielen Fallen zu einem Dauerschaden, der bei richtiger Dia- gnostik und adaquatem technischen Vorgehen vermieden werden kann.
