Die expli- zite Aufgabe der Anstalt ist von Anfang an die "Heilung der heilbaren und die Pflege und Verwahrung der unheilbaren Irren". Beim fruhen Stand der therapeutischen Kunst muBte wohl der zweite Aufga- ben bereich dominieren, schrieb doch Kraepelin in seinem Lehrbuch der Psychia- trie: "Allein eine genauere Kenntnis der Geistesstorungen lehrt uns, daB diesel- ben leider nicht nur immerschwere, sondern ihrer uberwiegenden Mehrzahl nach unheilbare Krankheiten darstellen. Wirklich ganz vollstiindige Heilungen im strengsten Sinn des Wortes sind verhaltnisrniiBig sehr selten . . . ". Das therapeutisch eingesetzte Spektrum von MaBnahmen war andererseits bereits damals recht weit gefachert. Es gab nach dem medizinisch-naturwissen- schaftlichen Verstandnis der Geisteskranken gestaltete medikamentose Behand- lungsversuche, wie Burian in seinem Beitrag zu diesem Teil des Readers umfas- sender darstellt, es gab die Suggestionsbehandlung, es existierten fruhe Formen der Psychotherapie. MaBnahmen von schockartigem Charakter, wie Drehstuhl, KaltwassergUsse etc. wurden zu diesem Zeitpunkt von den fruhen Klinikern be- reits ebenso abgelehnt wie MaBnahmen, die die Ruhigstellung der Kranken durch Einschrankung der motorischen Bewegungsfreiheit bewirken wollten, wie An- binden der Patienten oder die bekannte Zwangsjacke. Kraepelin meinte zu die- sem Thema, daB er die Anwendung der Zwangsjacke bereits 1907 seit nunmehr 17 Jahren nicht mehr erlebt habe; die Einschrankung der Bewegungsfreiheit der Kranken verrate das Niveau der Anstalt und der in ihr tatigen Arzte: "das mu- stergUitige Anstaltsleben kennt sie nicht mehr".
