People with learning disabilities and challenging behaviour are too often place in institutions and care homes far from home. These placements are often unnecessary, harmful and continue far longer than they should. People and families also do not get the chance to plan their own departure and organise good support back home in their own community.

This practical guide is based on decades of experience in successfully supporting people to move home and to build flourishing lives with family, friends, neighbours and colleagues. It is a travel guide that demonstrates tried and trusted methods for navigating the difficult road home. The guide includes:

  • Principles and values to guide you on your journey back home
  • Questions and processes necessary to develop a good plan
  • Systems and entitlements that you can use
  • Practical work required and examples of success
  • How to stay safe and well when you are in your own home

This guide is useful for people with learning difficulties, their families and professional or community allies who are supporting people on their journey home.
