Die Chirurgie der Brustorgane ist einer der jungsten Zweige der modernen Chirur- gie. Wahrend in der zweiten Halfte des vergangenen Jahrhunderts nach der Einfuhrung der Antisepsis und Asepsis namentlich in der Bauchchirurgie in kurzer Zeit gewaltige Fortschritte erzielt wurden, beschrankte sich die Tatigkeit des Chirurgen bei den Er- krankungen der Brusthohle lange auf die Behandlung von Brustfelleiterungen. Man stand damit immer noch auf der Stufe der hippokratischen Medizin, die schon dem Eiter durch Einschnitt in einen moglichst tiefen Zwischenrippenraum AbfluB zu ver- schaff en gewuBt hatte. Der Ausbau der Thoraxchirurgie war erst moglich, als gewisse Vorbedingungen erfullt waren. Dazu gehorte in erster Linie die Verbesserung der Diagnostik. Durch Perkussion und Auskultation war wohl die Erkenntnis der krankhaften V organge im Ilmern des Brustkorbes in vorher nicht gealmter Weise gefordert worden, aber erst die Rontgenuntersuchung erlaubte schon bei lebendem Korper ein Eindringen in die patho- logisch-anatomischen Vorgange, wie es fruher nur am Seziertisch moglich war, und wie es aber fur ein zielbewuBtes chirurgisches Handeln notwendig ist. Es war jedoch auch eine technische Frage zu losen, die nur der in ihrer ganzen Tragweite ermessen kann, der einmal einen Verletzten mit breit offenem Pneumothorax unter den Er- scheinungen hochster Atemnot hat sterben sehen: die Ausschaltung der Folgen des breit offenen Pneumothorax. Durch die Einfiihrung des DruckdifJerenzverJahrens durch SAUERBRUCH im Jahre 1904 wurde der eigentliche Grundstein fur die moderneThorax- chirurgie gelegt.
