Wir bemtihen uns in meiner Abteilung seit mehr als 20 1 ah ren, die chirurgische Laparoskopie, d. h. die Laparoskopie unter Beachtung speziell chirurgischer Fragestellungen, in unseren Kliniken und Abteilungen "hoffahig" zu machen. Erst in den letzten lahren wurde ihre Einbeziehung in die chirurgische Diagnostik der Abdominalerkrankungen bzw. Krankheitsprozesse der Bauchorgane erreicht. Und dies, ob gleich ein Chirurg, namlich Kelling, bereits 1901 die erste Spiegelung der Bauchhohle durchgefUhrt hat ("Kolioskopie beim Hund"). In meiner Abteilung konnten wir dieses Verfahren tiber die fUr die medizinischen Abteilungen unseres Krankenhauses zur Diagnose von Lebererkrankungen durchgefUhrten La paroskopien hinaus zur "Laparoskopischen Exploration der Bauchhohle" (H. Lindner und H. Henning) bei unklaren abdominalen Beschwerde-und Krankheitsbildern erweitern. Die Probelaparotomien wurden bei uns immer seltener, an ihre Stelle trat die unter gtinstigen Voraussetzungen auch ambulant durchfUhrbare, daher kostensparende und prak tisch risikoarme bis -freie "chirurgische Laparoskopie." Ausnahmeindikation fUr die Probelaparotomie bleibt vor aussichtlich u. a. das Staging bei Morbus Hodgkin. Die eindrucksvollen Fortschritte, die neue Untersuchungsver fahren wie Sonographie, Computertomographie, Nuklearme dizin etc. fUr die Differentialdiagnose der meisten Abdominal erkrankungen gebracht haben, schranken zweifellos in Zu kunft die Indikation zur chirurgischen Laparoskopie ein. Den noch konnen alle diese Verfahren die histologische Untersu chung von Gewebe, das nur durch Biopsie und Probepunktion gewonnen werden kann, nicht ersetzen. Das ist nur eine der In dikationen, die eine Weiterentwicklung der "chirurgischen La paroskopie" fUr die Differentialdiagnose und auch fUr einige therapeutische Bereiche (bioptische Blutstillung, Durchtren nung von Adhasionen etc.) unerlaBlich machen."
