Virale Infekte konnen die Schlagfrequenz deutlich herabmindern und sogar zum Erliegen bringen, aber auch abschwellende alpha-Adre- nozeptor-Agonisten (Xylometazolin) und moglicherweise auch einige in den Nasensprays haufig vorhandenen Konservierungsstoffe wirken hem- mend auf die Ziliartatigkeit. Daruber hinaus steigt bei Entzundungen die Viskositat des Nasensekrets erheblich an, zum einen infolge der entziind- lichen Exsudation und zum anderen als Folge des abnehmenden pH- abhangigen Hydratisierungsgrades der in dem Nasensekret gelosten EiweiBe (sekretorisches IgA, Glykoproteide, zellulare Enzyme etc.). Ziel einer effektiven Sinusitistherapie muB deshalb neben lokal abschwellenden MaBnahmen und der Beseitigung bakterieller Superin- fektionen auch die Wiederherstellung der gestOrten mukoziliaren Clear- VI Vorwort ance sein. Zur Anregung sowohl der Ziliaraktivitat des respiratorischen Epithels als auch der Sekretionsleistung der Becherzellen und der subepi- thelialen Drusen stehen zahlreiche definierte chemische als auch pflanz- liche Sekretolytika zur Verfugung.
