Die chronische Pyelonephritis entsteht definitionsgem� als Folge einer bakteriellen Infektion. Sie wird wegen der Art der morphologischen Organsch�den zur Gruppe der interstitiellen Nierenentz�ndungen gez�hlt. Es handelt sich offenbar um eine verh�ltnism� ig h�ufige Krankheit, da sie bei 40 der Kinder und bei etwa 20 der Er- wachsenen mit dialysepflichtiger Niereninsuffizienz als Grundleiden angenommen wird (112). Obwohl sich viele Un- tersuchungen mit dieser Erkrankung befa t haben, sind wesentliche Fragen der �tiologie, Pathogenese und Klinik noch ungekl�rt geblieben. So ist zum Beispiel noch v�llig offen, ob es beim Menschen ebenso wie im Tierexperiment eine chronische bakterielle Nephritis auch ohne die Einwirkung "pr�disponierender" Faktoren gibt (12,14,23,24,35,36,38,67,117,118,186,192, 194,293,298). Auch der Einflu einer bakteriellen Infektion auf die Form der morphologischen Ver�nderungen in der Niere und den Verlauf es Leidens ist umstritten (6,10,11,16,17, 26,37,75,77,84,90,95,103,121,131,157,179,199,200,202,203, 211,229,239,265,271,282,296,313,328,329,332,338,342,349, 350,352). Nach wie vor ist ebenfalls ungekl�rt, welche Me- chanismen daf�r verantwortlich sind, da auch in bakterio- logisch sterilen Nieren die Entz�ndung nicht ausheilt und in der chronischen Phase der Pyelonephritis zu einem allm�hlich fortschreitenden Verlust an Nierenparenchym f�hrt (58,59,84,95,131,182,228). Weiterhin herrscht Un- klarheit �ber die Bedeutung verschiedener Nierenfunktions- proben f�r Diagnose und Verlaufskontrolle der chronischen Pyelonephritis. Ein weiteres klinisch wichtiges Problem stellt die Beziehung zwischen chronischer Pyelonephritis und Hochdruckkrankheit dar. Bei etwa 20 % aller Hochdruck- kranken finden sich Hinweise auf eineNierenerkrankung. Die H�lfte dieser Nierenkranken soll an einer bakteriellen Nephritis leiden. Somit besteht die M�glichkeit einer kau- salen Beziehung zwischen chronischer Pyelonephritis und arterieller Hypertonie (2,37,48,83,105,129,148,219,263, 309,317,339,356,357).
