Gelenkerkrankungen partizipieren mit etwa 5-17% an allen Praxisdiagno- sen in der Bundesrepublik Deutschland. Sie kommen etwa 15mal haufiger als der Herzinfarkt und etwa 20mal haufiger als die Appendizitis vor. Wegen der mit ihnen verbundenen haufigen Arbeitsunfahigkeit und auch der Moglichkeit einer Friihinvaliditat besitzen Gelenkerkrankungen eine erhebliche sozialmedizinische Bedeutung. Nach Durchsicht verschiedener Statistiken ist anzunehmen, daB etwa 60% aller Gelenkerkrankungen klinisch und v.a. radiologisch primar unzu- reichend eingeordnet werden, wodurch sich einerseits eine mogliche effek- tive Therapie verzogern kann, andererseits die Voraussetzung gegeben ist, daB therapeutische Wege eingeschlagen werden, die das Grundleiden sogar verschlimmern konnen (z.B. durch Steroide). Aus dem Gesagten ergibt sich die Notwendigkeit nicht nur einer subtilen klinischen, sondern einer die Diagnose oft erst beweisenden sorgfaltigen Rontgendiagnostik. Eine solche Rontgendiagnostik ist auch fUr den Geiibten zeitaufwendig. Allein das Absuchen von iiber 30 Gelenkkonturen an einem Handskelett nach moglichen Erosionen kann schon einen Mindestzeitaufwand von etwa 5 min erfordern. Die Aufgabe des Rontgenuntersuchers bei der Abklarung von unklaren Gelenksymptomen darf nicht allein darin bestehen, von der Norm abwei- chende Rontgenbefunde zu konstatieren, er muB sie vielmehr synoptisch in die iibrigen klinischen und laborchemischen Befunde einfiigen. Da be- stimmte Gelenkerkrankungen, iiber rein morphologische Veranderungen hinausgehend, ein mehr oder weniger typisches Befallsmuster und eine be- stimmte Befallstopik besitzen, die al1ein schon von der Wahrscheinlichkeit her diagnoseweisend sein konnen, muB der Rontgenuntersucher, ausgehend von der Anamnese und den klinischen Erscheinungen, die Rontgenuntersu- chung der jeweils in Frage kommenden Gelenkregionen entsprechend pla- nen (s.S. XIV, f.).
