Seit dem Erscheinen der 4. Auflage des Handbuchs der inneren Medizin (Band IX/1) im Jahre 1960 sind 23 Jahre vergangen. Damals wie heute basiert das pathophysiologische Konzept der chronischen Herzinsuffizienz auf den phy- siologischen Grundlagen der Herz- Kreislauf- Dynamik, determiniert durch Vor- last, Nachlast, Kontraktilitat und Frequenz des Herzens in Verbindung mit den hamodynamischen, metabolischen und endokrinologischen Folgestorungen der Organperipherie (SCHWIEGK u. RIECKER 1960). Auch die von LINZBACH (1960) als "GefUgedilatation" beschriebenen Strukturveranderungen im Myo- kard vermitteln heute noch ein giiltiges morphologisches Paradigma. Neue Untersuchungsmethoden (z.B. Echokardiographie, Ventrikulographie, Coronarographie, Myokardszintigraphie, Myokardbiopsie) haben unsere Kenntnisse aber iiber die Nosologie der chronischen Herzinsuffizienz wesentlich vertieft und spezielle Verlaufsformen der Herzinsuffizienz (z.B. Herzvitien, pri- mare Kardiomyopathien, koronare Herzkrankheit, rhythmogene Herzinsuffi- zienz, chronische Perikarditits etc.) einer kausalen Therapie (z.B. durch herzchi- rurgische Ma13nahmen) zuganglich gemacht. Neben der konventionellen Glyko- sidtherapie haben neue inotrope Substanzen, Diuretika, Antiarrhythmika und Vasodilatantien und praventive Ma13nahmen (z.B. antihypertensive Therapie) das Spektrum der symptomatischen Behandlungsma13nahmen wirkungsvoll er- weitert. 1m Interesse der Aktualitat und Lesbarkeit wird in dem nun vorliegenden Handbuchband bewu13t auf eine inhaltliche Wiederholung der vorausgegange- nen Auflage verzichtet, ebenso auf eine umfassend liickenlose Themenfolge wie auch auf eine lehrbuchma13ige Wiedergabe dieses Gegenstandes; diese Monogra- phie zielt vielmehr auf eine selektiv vertiefende Darstellung vornehmlich der neueren Erkenntnisse der Pathophysiologie, Nosologie, Diagnostik und Thera- pie der chronischen Herzinsuffizienz mit besonderer Beriicksichtigung spezieller Verlaufsformen der Herzinsuffizienz und kausaltherapeutischer Gesichtspunkte.
