Decididos a reforzar sus sistemas de salud, los países se ven obligados cada vez más a reconocer que la perspectiva de la atención primaria de salud (APS) puede proporcionarles el nuevo y más definido rumbo que necesitan. En el análisis realizado en el Informe sobre la salud en el mundo 2008 se muestra que las reformas de la atención primaria, basada en los principios de acceso universal, equidad y justicia social, constituyen una respuesta fundamental a los retos sanitarios que plantea un mundo en rápida evolución y a las crecientes expectativas de los países y de sus ciudadanos en materia de salud y atención sanitaria.

El informe identifica cuatro conjuntos de reformas interrelacionadas de la APS que tienen por objeto lo siguiente: garantizar el acceso universal y la protección social, a fin de mejorar la equidad sanitaria; reorganizar la prestación de servicios en función de las necesidades y expectativas de la población; mejorar la salud de las comunidades mediante unas políticas públicas más idóneas; y renovar el liderazgo sanitario sobre la base de una mayor eficacia de las administraciones y la participación activa de los principales interesados directos.

Al mismo tiempo, está cambiando radicalmente la naturaleza de los nuevos problemas sanitarios. La urbanización, la globalización y otros factores aceleran la propagación mundial de las enfermedades transmisibles y aumentan la carga de trastornos crónicos. El cambio climático y la inseguridad alimentaria tendrán grandes repercusiones en la salud en los años venideros, de modo que la articulación de una respuesta eficaz y equitativa entrañará enormes retos.

Ante todo eso, seguir haciendo lo habitual no es una opción viable para los sistemas de salud. Muchos de ellos parecen hallarse a la deriva, fijándose una tras otra prioridades a corto plazo, de forma cada vez más fragmentada y sin un sentido claro de la preparación necesaria para afrontar el futuro.

Por fortuna, el actual contexto internacional es favorable a una renovación de la APS. La salud mundial acapara un interés sin precedentes. Se aboga cada vez más por la unidad de acción y abundan los llamamientos en pro de la atención integral y universal y de la salud en todas las políticas. Las expectativas nunca habían sido tan altas.

Aprovechando este impulso, la inversión en reformas de la atención primaria puede transformar los sistemas sanitarios y mejorar la salud de los individuos, las familias y las comunidades en todas partes.
