Nach ausgedehnten Tierversuchen fand das Ver- fahren Eingang in die Klinik und eroberte sich in Kürze einen festen Platz im Gesamt- spektrum der verschiedenen lokalen Konservierungsmethoden. Die konservative Nierenchirurgie erhielt durch die Anwendung der Ballonokklusion mit kontrollierter Kälteperfusion eine neue Dimension. Der breitgefächerte Anwen- dungsbereich reicht von den Steinerkrankungen über die doppelseitige Tumorbildung bis zu den komplizierten plastischen Korrekturen am Kelchsystem. Die enge Korrelation zwischen Ischämiedauer und drohendem Funktionsverlust setzte bisher dem Eingriff am Nierenparenchym natürliche Grenzen. Der unter Zeit- not stehende 0perateur war gezwungen, zwischen sorgfältiger Technik und irrever- sibler Parenchymschädigung einen zweifelhaften Kompromi zu schlie en. Die risiko- lose Verlängerung der Ischämiedauer ist mit einer der wesentlichen Vorteile dieses Verfahrens. Bei einer minimalen Komplikationsquote beweisen die differenzierten postoperativen Funktionsprüfungen die schnelle Erholungsfähigkeit des Parenchyms bis zu den ursprünglichen Ausgangswerten. Die moderne konservative Nierenchirurgie stellt an den Operateur nicht nur hohe technische Anforderungen, sie erfordert auch Kenntnisse auf dem Gebiet der Nieren- physiologie sowie Bereitschaft und Möglichkeit der interdisziplinären Zusammenarbeit mit Radiologen und Nuklearmedizinern. .
