Les infections de prothèses de hanche à staphylocoques sont-elles toutes identiques? Cette étude sur 53 infections de prothèses de hanche à staphylocoques a permis de montrer deux profils évolutifs bien distincts. Les patients qui ont eu une infection à staphylocoques sur une prothèse de hanche traumatique ont un taux de mortalité très élevé et un taux de guérison médiocre, alors que dans le cas d'une prothèse non traumatique, le taux de guérison est bien supérieur. Les Staphylococcus epidermidis sont le plus souvent multi résistants et donnent des infections torpides à cause de leur particularité à créer un biofilm. Les infections à Staphylococcus aureus résistants à la méticilline(SARM) représentent 30% des infections à Staphylococcus aureus et sont retrouvées très fréquemment chez les patients fragiles ou dans les infections polybactériennes. La totalité des SARM sont actuellement résistants à l'ofloxacine. Un nombre élevé de prélèvements peropératoires positifs est corrélé à l'innoculum bactérien articulaire et au risque d'échec du traitement médico-chirurgical. L'infection de prothèse de hanche à staphylocoques nécessite donc une prise en charge multidisciplinaire.
