Bei der Betrachtung von Verletzungen und Verletzungsfolgen im Bereich der Hal tungs-und Bewegungsorgane standen jahrzehntelang die Skelettveranderungen, die Frakturen, auch unter dem Eindruck der Entwicklung zahlreicher neuer Osteosyn theseverfahren, ganz im Vordergrund. Insbesondere bei Arbeits-und Verkehrsunfal len wurden Verletzungen der sog. Weichteile iiberwiegend als Begleitverletzungen von Knochenbriichen beobachtet; und sie schienen von untergeordneter Bedeutung. Lediglich gedeckte und offene Sehnenverletzungen hatten sowohl als Begleitverlet zungen wie als isolierte Verletzungen von jeher einen eigenen Stellenwert. Diese Einschatzung der Weichteilverletzungen hat sich gewandelt. So sind in den letzten Jahren einerseits Erkenntnisse gewonnen worden, die die Bedeutung von begleitenden Weichteilverletzungen, Z. B. bei Frakturen, neu definieren. Andererseits ist, insbesondere unter dem Eindruck eines geanderten gesellschaftlichen Freizeit verhaltens -mit Zunahme der sportlichen Aktivitaten im Breitensport und Maximie rung der Leistungsanforderung im Spitzensport -, auch eine auBerordentliche Zunahme von isolierten Weichteilverletzungen zu verzeichnen. Das trifft ganz beson ders fUr Muskelverletzungen zu. Unter den Sportverletzungen sind Muskelverletzungen mit einem Anteil von 20 -30 % enthalten, als Kontusionen, Muskelfaserrisse und Muskel-Teilrupturen bis hin zur kompletten Muskelruptur. Die Differenzierung derartiger Verletzungen beruht heute nicht mehr nur auf dem klinischen Eindruck; sie laBt sich vielmehr durch bildgebende Verfahren objektivieren; dadurch sind auch die Moglichkeiten der Friiherkennung und Friihtherapie deutlich verbessert. Zudem ist die bis heute viel fach vertretene Ansicht, eine Regeneration von Muskelfasern nach Verletzung sei nicht moglich, jede Muskelwunde heile nach Organisation und Resorption eines Rup turhamatoms bindegewebig aus und die verbleibende Muskelnarbe fUhre zu einer funktionellen Minderwertigkeit, aufgrund zahlreicher neuerer experimenteller Untersuchungsergebnisse iiberpriifungsbediirftig geworden."
