Die schwere unfallbedingte Verletzung stellt sowohl fiir den Patienten als auch fur die Gesellschaft eine auBerordentliche Belastung dar. Der Verunfallte ist betroffen durch eine in der Regel lange Krankenhausbehandlung und Rehabilitationsphase, verbun- den mit der Belastung seiner sozialen Bindungen und eventuellen korperlichen und psychischen Langzeitschiiden, die seine Arbeitsfahigkeit vermindern und bis hin zur Pflegebedurftigkeit reichen konnen. Das Gesundheitssystem wird durch enorme Akutbehandlungskosten, welche bis weit uber 100000 DM betragen konnen, und die weiteren Folgekosten fiir die Rehabilitation belastet. Volkswirtschaftlich sind jedoch diese Aufwendungen gerechtfertigt, da bei einem Durchschnittsalter der polytrau- matisierten Patienten von ca. 30 Jahren ungefahr 80% der Oberlebenden in die Berufstatigkeit zuruckkehren. 1m Rahmen der medizinischen Versorgung stellt das posttraumatische Organver- sagen nach schwerer Unfallverletzung mit einer hohen Sterblichkeitsrate von 10 - 30 % hochste Anforderungen. Deshalb kommt neben der Verbesserung der organerhaltenden MaBnahmen insbesondere der Prophylaxe und der Vermeidung von Sekundarschiiden eine ganz besondere Bedeutung zu. Hierzu ist einerseits eine kontinuierliche Verbesserung der Rettungskette und gerade auch der organisatori- schen Strukturen im Krankenhaus bei der Erstversorgung notwendig. Andererseits muss en die Ergebnisse pathophysiologischer und pathobiochemischer Grundlagen- forschung in den Behandlungsalltag eingebracht und auf ihren praktischen Wert untersucht werden. Dafiir ist eine enge Zusammenarbeit und ein standiger Wissens- und Erfahrungsaustausch zwischen Grundlagenforschern und den an der Patienten- versorgung beteiligten Fachdisziplinen unabdingbar. Die Beitrage im vorliegenden Buch stellen deshalb die aktuellen Moglichkeiten zur Vermeidung und Behandlung des posttraumatischen Multiorganversagens interdis- ziplinar dar. 1m Vordergrund stehen dabei neue Behandlungskonzepte sowie der therapeutische Stellenwert neuer pathophysiologischer Erkenntnisse.
