Zur Beantwortung der Frage, wann und unter welchen Bedingungen eine antibakterielle Therapie indiziert ist, bedarf es der Vorkliirung einer Reihe von Pro- blemen. Sie betreffen den Wandel bakterieller Infektionen, die Ursachen des Hospita- lismus, die organisatorischen und baulichen MaBnahmen im Rahmen der Krankenhaus- hygiene etc. Therapeutische Entscheidungen vedangen in gleicher Weise Kenntnisse Uber die unterschiedlichen Wirkungsmechanismen der Antibiotika und ihre Pharmako- kinetik unter normalen und pathologischen Bedingungen. Die Technik der Material- gewinnung, die Regeln fUr die Zusammenarbeit der Klinik mit dem Mikrobiologen, die Aussagekraft und Bedeutung von Antibiogrammen soIlen hier nur als wichtige Teilprobleme genannt werden. FUr den klinischen Bereich gibt es allgemein giiltige Grundregeln, die die Indikations- stellung, die Auswahl, die Kombination und Dosierung der Antibiotika betreffen. In allen Spezialdisziplinen liegen dariiber hinaus spezielle Probleme vor, die bei jeder Chemotherapie zu beachten sind. Zu dies en Fragen auBerten sich die Kliniker, unter- stiitzt von Mikrobiologen, Hygienikern und Pharmakologen, mit dem Ziele, in Empfeh- lungen festzulegen, bei wem, unter welchen Bedingungen, wie und unter Einsatz welcher Mittel eine antibiotische Therapie sinnvoll erscheint, aber auch welche Risiken und Nebenwirkungen einzukalkulieren sind. Ein ganz spezielles Problem zog sich wie ein roter Faden durch alie Referate und die Diskussion: die Entstehung und Therapie bakterieller Infektionen bei Intensivtherapiepatienten.
