Der Zugang zum tiefen Rektum und zum Analkanal durch direkte Spaltung des gesamten Sphinkterapparates wurde vor über 100 Jahren durch Cripps erfolgreich angewendet und von ihm eine entsprechende Serie von 36 Patienten veröffent- licht. Cripps war dabei angenehm überrascht, trotz fehlender 2 Rekonstruktion bei über j der Patienten anlä lich von Spät- 3 kontrollen gute Kontinenz zu finden. Die Möglichkeit geriet aber in der Zwischenzeit weitgehend in Vergessenheit, und im "chirurgischen Armamentarium" hat sich eigentlich nur die parasakrale Rektotornie nach Kraske gehalten, welche den Sphinkterapparat möglichst unbehelligt lä t. Es ist das Verdienst von Y ork Mason, die mannigfachen Möglichkeiten des sphinkterspaltenden Zugangs erneut und überzeugend dargelegt und auf die ausgezeichnete Kontinenz- funktion bei adäquater Rekonstruktion hingewiesen zu ha- ben. Nachdem wir uns von der Nützlichkeit des sehr pragma- tisch durchgeführten Eingriffs überzeugt hatten, schien es uns unbedingt erforderlich, die anatomischen Voraussetzungen eines möglichst blutsparenden Vorgehens zu präzisieren und dieses Vorgehen in Beziehung zu setzen zu den heutigen Kenntnissen der normalen Kontinenz respektive Defäka- tionsfunktion. Herr Huber hat sich - beraten durch den Vorsteher des Instituts für klinische Anatomie im hiesigen chirurgischen De- partement, Herrn Prof. A. von Rochstetter- anhand frischer Dissektionspräparate über ein Jahr mit den Fragen dieses Zugangs befa t. An den klinischen Fällen dieser Zeit konnten dann seine Erkenntnisse immer wieder in ihrer Bedeutung für die chirurgische Praxis überprüft werden. Die entspre- chenden gemeinsamen Erfahrungen sind in der kleinen Mo- nographie niedergelegt.
